General information for all miscellaneous consular service/application :

Step 1: Visit the website eSewa: https://indianconsularservices.mea.gov.in/consularServices/ . First, the applicant has to register on the portal. Then login and apply for the desired service(s) and upload the required documents on the portal and submit the application online. Applicants are advised to ensure that all the information is correct. It is also advised to keep the scans of the documents ready as per the checklist. Each file size should be less than 200kb.

Step 2: Embassy will scrutinize/check the application and documents online.  Then, applicant will receive a notification in this regard. In case of objection while checking, the same will be intimated to the applicant through the online portal. In this case, the applicants are required to re-submit the application after uploading the relevant documents. The Embassy will again check the application and intimate the status though the same online portal.

Step 3: After online verification of documents by the embassy, the applicants are required to visit the Embassy after booking an appointment at https://www.indianembassywarsaw.gov.in/apt/appointment.php . The applicant must ensure to bring printed online application form along with one set of copies of all the documents which are uploaded online, as well as the original documents for verification.

Step 4 : After physical verification of original documents at the embassy, fee will be accepted in the embassy. Check Fees .  Fee is accepted by card only.

Step 5 : The documents are usually issued on the same day and can be collected at 4PM.

Note:  i. Applicants are advised to visit the Embassy only after the approval of application by the Embassy through online mode(eSewa). 

ii. Please note that no email or sms is received for appointment confirmation. You can take printout/screenshot of appointment confirmation at https://www.indianembassywarsaw.gov.in/apt/appointment_search.php .

iii. If you wish to reschedule/cancel your appointment, first you need to cancel the appointment at https://www.indianembassywarsaw.gov.in/apt/appointment_search.php , and then, book a new appointment again at https://www.indianembassywarsaw.gov.in/apt/appointment.php

Please note that the Embassy of India in Warsaw does not issue:

- Indian Driving Licences, International Driving Permits, Aadhar Cards, PAN cards, Voter ID card.

-  Embassy does not issue Bachelorhood, Spinsterhood or marriage clearance (or no impediment) or any other certificate in this regard. A Certificate of bachelorhood/Single-status/eligibility to get married can be issued by a competent court/SDM having jurisdiction over the area of the applicant's residential address. This can be in the form of a certificate or affidavit notarized by the SDM, which is required to be counter-attested by the Home Department of the State concerned. Thereafter, it is to be attested/apostilled by the Attestation Section of the Consular, Passport & Visa (CPV) Division of the Ministry of External Affairs, New Delhi.

- Apostillation Service: Apostillation Service is not available at the Embassy. For information on apostillation of documents, please visit https://www.mea.gov.in/apostille-menu.htm 


IMPORTANT NOTICE

Embassy of India, Warsaw has received Indian driving licences from Polish Transport Authority.  Indian nationals whose licences were kept by the Polish Transport office, while issuing Polish driving licences, may contact the Embassy on inf.warsaw@mea.gov.in .

- Self-attested copy of Polish TRC/Visa and original (for verification at embassy). - Original POA document and its copy.

- Copy of one ID (Aadhar/Passport etc) of attorney

- Copy of passports(first and last page) of two witnesses

- Copies of Aadhar Card and PAN Card (if mentioned on the power of attorney)

Note: The particulars in the power of attorney should match with the passport of the applicant i.e. Given Name followed by Surname.

- Applicant and both witness (Indian citizens) should visit the embassy.

- Please note- POA document can be printed on plain paper (A4 size). Stamp Fee can be paid in India after attestation from Embassy. 50% space on the bottom of last page should be empty for photo of the applicant and signature/stamps by the Embassy.

Certificate in connection with applying for Polish Driving licence

Application for reissuance of International Driving Permit






- Miscellaneous form duly filled in with passport size photo pasted on the form.  Form must be signed as well by the applicant.

- Self-attested copy of Polish TRC/Visa and original (for verification at embassy). Please note : After due processing of the application, the Embassy will issue a Receipt to the applicant. Applicant will then need to apply for renewal of IDP through the portal www.parivahan.gov.in and upload all required documents, including the receipt issued by the Embassy.  IDP will be reissued by the licensing authority, i.e. Ministry of Transport and Highways, Government of India and it will be sent directly to the address of the applicant.  All any other query regarding IDP, please visit www.parivahan.gov.in.

Note: Please note that if applying for true copy of education documents, marriage/birth certificate etc, the document should be apostilled from Ministry of External Affairs, New Delhi.

Death Certificate 

(i) For Transportation of body to India, the following documents are required to be submitted to the embassy :-

- Original Death Certificate issued by the Hospital with English Sworn Translation and copy of both.

-  Death certificate issued by the Polish authority (with an apostille by the Polish Foreign office)

- Embalming certificate and its Sworn Translation in English

- The original passport of the deceased, and it's photocopy

- Authorization to transport the ashes to India (with Sworn Translation in English) issued by the District Sanitary and Epidemiological Station of the City of Warsaw (at ul. Kochanowskiego 21) .  In order to get this document, you are requested to share the information that  to which city and state the body will be transported. Thereafter, Embassy will issue a document which is required to be submitted to the District Sanitary and Epidemiological Station.

All the above documents are required to be sworn translated to English (except Passport).

On receipt on the above documents, Embassy will issue Death Certificate, Non-Covid Certificate, Transportation Letter.


(ii) In cases, where the family chooses to cremate the body in Poland and transport ashes to India, the following documents are required to be submitted to the embassy :-

- Original Death Certificate issued by the Hospital with English Sworn Translation and copy of both.

- Death certificate issued by the Polish authority (with an apostille by the Polish Foreign office)

-Cremation certificate and its Sworn Translation in English

- The original passport of the deceased, and its copy

-  Copy of the passport of a person responsible to carry the ashes

-   Authorization to transport the ashes to India (with Sworn Translation in English) issued by the District Sanitary and Epidemiological Station of the City of Warsaw (at ul. Kochanowskiego 21) .  In order to get this document, you are requested to share the information that  to which city and state the body will be transported. Thereafter, Embassy will issue a document which is required to be submitted to the District Sanitary and Epidemiological Station.
