It is necessary to register the birth of an Indian child, born abroad, with the Indian Embassy / Consulate in the country of birth. Such registration should normally be done within one year of the birth. The Embassy / Consulate issues a birth certificate to the child, which entitles the child Indian citizenship by descent.

Take a print of the application, and present the application to the Indian Embassy along with supporting documents and the fee.
 
Supporting Documents:
(i) Birth Certificate of the child issued by competent authority in country of birth i.e. Portugal. 
(ii) Copies of the passports of both the parents.
(iii) A copy of the certificate of Indian Citizenship of parents, if they are not citizen of India by birth. 
(iv) A copy of the marriage certificate of the parents. 
(v) A declaration letter by parents that the child does not hold passport of any other country.
